Registration Guidelines

On-line registration is closed.

Upon submitting the Registration Form, you will receive a PRN (Participant Registration Number) by e-mail to be included in payment details. Please do not send any payments without PRN included in payment details. Such a payments cannot be identified.

All participants are kindly requested to register in advance by completing and submitting the Registration Form with the appropriate fees.

Registration Fees

Please note unpaid pre-registration will not be considered as a valid registration unless fully paid and confirmed. Pre-registration summary therefore cannot be taken as a valid registration confirmation.
Please make sure to pay your registration fee within the time frame for early or late registration.

Registration fee include:

• Admission to all sessions, poster area and exhibition
• Admission to a "Welcome Reception“
• Symposium bag including Abstracts - Supplement to Hematologica journal, Final program, City map,
• Name badge + voucher for tours (if required and paid for)
• Coffee breaks
• List of participants

All Participants need to re-confirm attendance of all functions in their Registration Form.


Upon receipt of the correct registration fee, each participant will receive a confirmation of registration. 
Please bring this confirmation to the Registration desk at Diplomat hotel
as proof of your registration. Confirmation will be sent upon receipt of payment.

Please note that registration for the various tours and events will be confirmed upon payment on a “first come, first served” basis and numbers may be restricted.


All fees for the symposium registration should be paid in EUR (€). In case you are paying via bank transfer please note that we must receive the full amount clear of all bank charges except of payments done by European Union citizens or in EU based institutions for which SHA payment conditions applies.

Please include PRN (Participants registration number) as variable symbol to identify your payment. Please, forward a copy of the transfer stating the participant’s name.
After October 31, 2012 you are kindly requested to make all payments by credit card exclusively or to register on-site. Payment by bank transfer will no longer be available by then.

Cancellation / Refund Policy

All cancellations must be notified in writing (by mail, fax or e-mail) to the symposium secretariat according to the following conditions. Please note cancellations received:


 Until August 15, 2012                                                € 40 will be withheld for administrative fees
From August 15, 2012 till September 30, 2012 Reimbursement of 50% of the fees paid
As of October 1, 2012 No refund can be processed






Registrants may transfer their registration to a colleague at any time. For any change of name a fee of € 30 will be charged. 
All registrations received at the symposium secretariat - CZECH-IN, by October 26, 2012 will be included in the list of participants.

The participant acknowledges that he/she has no right to lodge damage claims against the organisers should the holding of the symposium be hindered or prevented by unexpected political or economic events or generally by force majeure, or should the non-appearance of speakers or other reasons necessitate programme changes. 
With registration, the participant accepts this proviso. 

Consent to the processing of personal data

I agree to the collection, preservation and processing of personal data contained in this form, by the administrator CZECH-IN s.r.o., Prague Congress Centre, 5. kvetna 65, Prague 4, 140 21 (or its employees) for the purpose stated below.
This consent is granted for all of the information contained in this form, and throughout the 5 (five) years from the date of consent. At the same time I am aware of / and of their rights under § 12 and 21 of Act No. 101/2000 Coll. on the protection of personal data.

Purpose of processing personal data

In accordance with § 5 of Act No. 101/2000 Coll. on the protection of personal data, all information given on this form is collected and processed solely for purposes of processing registration and accommodation requirements for the 6th International Symposium on MDS and Bone Marrow Failure Syndromes in childhood 2012 and for sending commercial messages to the data subject via electronic means pursuant to Act No. 480/2004 Coll. This agreement should not exceed 5 years and cancelled at any time upon request.

Summarized data from this form may be used by the administrator for statistical purposes and for internal use by the administrator.

Statement by Administrator

The data administrator declares that they will collect personal data to the extent necessary to accomplish the intended purpose and may be processed only in accordance with the purpose for which they were collected. Employees of the administrator or other individuals who process personal data under contract with the administrator and other persons are obliged to maintain the confidentiality of personal data, even after termination of employment or work.


The registration fees do not include insurance of participants against accidents, sickness, cancellation, theft, property damage or loss. Participants are advised to take out adequate personal insurance
